HEALTH BENEFITS
The Teamsters Local Union No. 155 Health Benefits Plan is funded under the terms of collective agreements negotiated between Teamsters Local Union No. 155 as part of the BC Council of Film Unions, and motion picture and television production companies, represented by the Alliance of Motion Picture & TV Producers. It provides benefits to eligible Teamsters Local Union No. 155 bargaining unit employees of participating producers.
DRIVER'S MEDICAL EXAM
To claim reimbursement for a Driver’s Medical Exam:
- Complete the Standard Health Claim Form and write anywhere on the form “EMPLOYMENT-RELATED EXAM’
- Keep copies of items you submit to Pacific Blue Cross for your records or follow-ups
- Mail or drop off the signed form with receipt to Pacific Blue Cross to the addresses at the top of the form
Please give Pacific Blue Cross approximately two weeks before you follow-up on a claim. Have your policy and I.D. numbers handy if you need to follow up.
PENSION
The Teamsters Local Union No. 155 Pension Plan has been established effective January 1, 2004 to provide retirement pensions to eligible members working in the bargaining unit of Teamsters Local Union No. 155. It is funded under the terms of collective agreements negotiated between Teamsters Local Union No. 155 as part of the BC Council of Film Unions, and motion picture and television production companies, represented by the Alliance of Motion Picture & TV Producers.
